Chronic Conditions Support Program - Toolkit

What is the Tookit?

The health professionals involved in the CCSP use a computer-based Toolkit. This electronic database allows them to collect, store and share information about your condition with each other. The Toolkit is provided by the Ministry of Health in BC and Yukon has an agreement to use it.
The Toolkit helps your health professionals keep a close eye on test results for your condition and reminds them when to call you in for new tests according to the timelines in the Canadian guidelines.

What Information is in the Toolkit?

The database stores information about your condition or conditions – your identification, test results and examination results. It helps your health professionals monitor changes in test results and work with you to improve treatment.

Who Sees the Information in the Toolkit?

The doctor or nurse responsible for your care decides who will access the information in the Toolkit. They will share information about your condition with those health professionals in the Chronic Conditions Support Program who are involved with your care, as needed. For example: your doctor or nurse may share information with the dietitian at the hospital. The Toolkit makes it possible for them to share information electronically.

Health and Social Services also uses the information to look at how care is being improved overall for people with chronic conditions. To ensure privacy, names and other identifiers are not attached to this information. It helps identify trends and areas of improvement, such as whether more people are getting tests at the right times.

How is Personal Information Protected?

  • Your consent is required to collect, use or disclose your personal information for any purpose other than those stated above.
  • Only authorized individuals directly involved in this initiative have access to it. Security safeguards are in place to ensure access is restricted to these authorized individuals.
  • We strive to keep your personal information accurate and up to date.

Opting Out

You have the right not to be included in the program if you choose. Opting out of the program will not limit or prevent you from receiving care.

If you opt out of the program, your health care providers cannot make use of the Toolkit for such things as electronic reminders, to recall you for tests, or for sharing information. If you opt in (consent) and later change your mind, just contact your doctor’s office or your health centre to let them know.

Authorization to Collect Personal Information

The collection of patients’ personal information by health care practitioners is authorized under the Health Act, in order to provide ongoing care and treatment.
